Reliq Health Technologies (RHT.V or RQHTF) has built a comprehensive hardware and software solution that allows patients to receive high quality care in the home, improving health outcomes, enhancing quality of life for patients and families and reducing the cost of care:
· Commercial stage technology w/ 2 pilot projects in the US
1. Feldman Institute in Baton Rouge, Louisiana: a 3 facility pain clinic that treats thousands of patients annually.
2. A paid pilot project w/ Florida based Sacred Heart Health System (a five hospital system). Sacred Heart will evaluate the use of Reliq’s CareKit Health platform to monitor patients post-discharge from hospital. Paid pilot (we cannot disclose amount), however, we expect to sign a contract for a full-scale deployment in Q4 2016.

Admitted over 30,000 patients
Performed over 15,000 surgeries

15% of Sacred Heart’s patients are admitted and/or receive surgery for cardiac issues, which is the primary target of our pilot with them
Sacred Heart is a part of Ascension Health – the nations largest system of Catholic and non-profit health care facilities
We expect that in 2017 a minimum of 5% of all 30,000 inpatients discharged from Sacred Heart will be monitored in the home using the CareKit platform, representing US$3.6 Million in annual revenue for Reliq. That number will grow as Sacred Heart rolls out their post-discharge care to multiple patient populations (not just CHF but also oncology, post-surgical, diabetes, COPD, palliative care, dementia, etc.).
3. Won a USD1.22M contract with Nexgen out of San Antonio, Texas. The majority of this contract will be realized during 2016 with a potential to expand this further. This contract will turn Reliq into a
cashflow positive Company in Q2 2016 and the Company won’t need any further capital raises unless for acquisitions and won’t be at current valuation levels.
